Welcome to NuLera Medical. These Terms and Conditions govern your use of our website, booking platform, and services. By accessing this website, scheduling an appointment, purchasing services, or otherwise engaging with NuLera Medical, you agree to be bound by these Terms and Conditions.
If you do not agree to these Terms and Conditions, please do not use this website or book services with NuLera Medical.
1. Practice Model
NuLera Medical is a direct-to-patient cash-pay practice. We do not bill insurance for visits, services, or laboratory-related charges.
By booking with NuLera Medical, you understand and agree that:
All services are self-pay.
Payment is required before an appointment may be booked.
You are personally responsible for all fees charged by NuLera Medical.
You are personally responsible for all laboratory fees, if labs are ordered.
NuLera Medical does not submit claims to insurance for visits, services, or labs.
Insurance cannot be billed by the practice, and patients should not expect reimbursement from insurance for visits, services, or labs.
2. Not Primary Care
NuLera Medical is not a primary care practice and does not function as your primary care provider. Services offered through NuLera Medical are intended to complement, not replace, routine primary care.
By using this website or booking services, you understand and agree that you are responsible for maintaining an ongoing relationship with a primary care provider for preventive care, general medical care, chronic disease management, routine screenings, urgent concerns, and continuity of care.
NuLera Medical does not assume responsibility for comprehensive primary care services, full coordination of all aspects of your medical care, or management of conditions outside the scope of services specifically provided by this practice.
3. Payment Terms
Full payment is due in advance of booking unless otherwise stated in writing by NuLera Medical.
By scheduling an appointment, you authorize NuLera Medical to charge the payment method you provide for any fees owed under these Terms and Conditions, including appointment fees, cancellation fees, no-show fees, and any other outstanding balances.
If any payment is declined, disputed, reversed, or otherwise unpaid, NuLera Medical reserves the right to refuse future booking until the balance is paid in full.
4. Appointment Booking
Appointments are not confirmed until payment has been received and the appointment has been accepted by NuLera Medical.
NuLera Medical reserves the right to reschedule, refuse, or cancel appointments at its discretion, including but not limited to situations involving nonpayment, inappropriate conduct, suspected misuse of services, or scheduling conflicts.
5. Cancellation and No-Show Policy
We value reserved appointment time and ask that patients respect the scheduling needs of the practice.
Cancellations made with less than 24 hours’ notice will incur a $50 cancellation fee.
A no-call, no-show will incur a $50 fee.
If a patient wishes to rebook after a late cancellation or no-call, no-show, the $50 fee must be paid first before another appointment may be scheduled.
Due to limited appointment times and lost revenue, there are no exceptions to our cancellation and no-show policy.
6. Refund Policy
All sales are final.
There are no refunds after appointments.
Fees paid for completed services, provider time, consultations, evaluations, or appointments already rendered are nonrefundable.
Missed appointments, late arrivals, or unused appointment time do not entitle the patient to a refund.
Refunds for appointments may be issued if requested more than 24 hours in advance of the appointment.
7. Laboratory Testing
If laboratory testing is recommended or ordered:
Lab fees are separate from appointment fees unless explicitly stated otherwise.
Patients are solely responsible for all lab fees and related charges.
NuLera Medical does not bill insurance for labs.
Patients should not expect insurance reimbursement for labs ordered through NuLera Medical.
Patients are responsible for confirming any outside payment arrangements directly with the laboratory, if applicable.
8. Patient Financial Responsibility
By using this website or booking services with NuLera Medical, you agree that you are financially responsible for all charges associated with your care, including but not limited to:
consultation fees
appointment fees
follow-up fees
late cancellation fees
no-show fees
laboratory fees
other charges disclosed before service
Failure to pay outstanding balances may result in refusal of future services until the account is brought current.
9. No Guarantee of Outcome
NuLera Medical provides individualized medical and wellness services, but no specific diagnosis, outcome, response, or result can be guaranteed.
Information provided on this website or during services is for informational, educational, and clinical purposes only and should not be interpreted as a guarantee of results.
10. Not Emergency Care
NuLera Medical does not provide emergency services.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Do not use this website, email, text, portal messages, or appointment requests for urgent or emergency medical concerns.
11. Website Content and No Provider-Patient Relationship by Website Use Alone
The information on this website is provided for general informational purposes only and does not create a provider-patient relationship by itself.
A provider-patient relationship is established only after NuLera Medical has accepted you as a patient and appropriate intake, consent, and clinical requirements have been completed.
12. Privacy and Communications
By using this website or booking services, you consent to communications from NuLera Medical related to scheduling, billing, reminders, operational matters, and care coordination, as permitted by applicable law.
Use of this website is also subject to any applicable Privacy Policy, Notice of Privacy Practices, consent forms, and office policies issued by NuLera Medical.
13. Acceptable Use of Website
You agree not to misuse this website or its content. You may not:
copy, reproduce, or republish website content without permission
use this website for unlawful purposes
interfere with the security or operation of the website
submit false, misleading, or fraudulent information
NuLera Medical reserves all rights in its website content, branding, logos, text, and materials unless otherwise stated.
14. Changes to These Terms
NuLera Medical may update these Terms and Conditions at any time without prior notice. The most current version will be posted on the website with the updated effective date.
