NuLera LLC (doing business as NuLera Medical, "we," "us," "our") operates nuleramedical.com and the NuLera Medical patient portal. This Privacy Policy explains what information we collect, how we use it, and your rights regarding it. By using our website, booking an appointment, or providing your contact information to us, you agree to this policy.
Information we collect
We may collect contact details (name, date of birth, address, phone number, email), health information you provide during intake or treatment (medical history, medications, allergies, treatment goals), appointment, payment, and visit history, photographs taken with your written consent (per our Photography Consent), and standard web analytics (device type, browser, IP address, pages viewed).
Health information collected during treatment is protected under HIPAA. Our separate Notice of Privacy Practices explains your rights regarding your protected health information. Ask in person or email us for a copy.
How we use your information
We use the information we collect to schedule, deliver, and follow up on your healthcare appointments, send appointment reminders, confirmations, and post-visit follow-ups, process payments and deposits, comply with state and federal healthcare regulations, and improve the quality of our care and our website.
SMS / text message communications
We send the following types of text messages to phone numbers patients provide us: appointment booking confirmations, appointment reminders (typically 48 hours and 24 hours before a visit), reschedule and cancellation notices, post-visit follow-up messages, and occasional service announcements that you may opt out of separately.
How patients opt in: when you book an appointment with NuLera Medical or sign up for our patient portal, you provide your phone number on the booking or signup form. The form clearly states that by providing your number, you consent to receive the messages described above. Consent is given by you actively typing your phone number into the form and submitting it; we do not pre-select consent on your behalf.
Message frequency varies based on appointment activity (typically 1 to 4 messages per appointment cycle). Message and data rates may apply.
You may opt out of text messages at any time by replying STOP to any message you receive. Reply HELP at any time for assistance, or email us at hello@nuleramedical.com.
We do not share your mobile information, including your phone number, with third parties or affiliates for marketing or promotional purposes. Mobile information is shared only with subprocessors who help us deliver our services (such as our SMS provider) and only as needed to send you the messages described above.
Information sharing
We do not sell your personal information. We share it only with healthcare providers involved in your care, with service providers who help us run our practice (SMS, email, payments, scheduling, electronic health records) under contracts requiring confidentiality and HIPAA compliance where applicable, when required by law (subpoena, court order, public health reporting), or with your written authorization.
Your rights
You have the right to access your records and request copies, request corrections to inaccurate information, request deletion (subject to medical record retention laws), withdraw consent for non-essential communications at any time, and opt out of marketing or promotional messages. Information security
We protect your information using encryption, access controls, and physical safeguards. No method of transmission is 100% secure, but we take reasonable steps to protect your information from unauthorized access, use, or disclosure.
Children
Our services are not directed at children under 13, and we do not knowingly collect information from children.
